Phase 3 obesity evidence
TRIUMPH-1 reported mean weight loss of 25.0% at 12 mg versus 3.9% with placebo at 80 weeks.
Context: 2,339 adults with obesity; phase 3 randomized trial.
Limitations: Trial methods are research context. No split-weekly regimen or peptide combination was validated by this trial.
Route: SC · Human SC administration; absolute percentage not established here.

Phase 2 obesity efficacy at 24 and 48 weeks
In 338 randomized adults, least-squares mean weight reduction at week 24 was 7.2% (1 mg), 12.9% (combined 4 mg), 17.3% (combined 8 mg), and 17.5% (12 mg), versus 1.6% placebo; at week 48: 8.7%, 17.1%, 22.8%, and 24.2%, versus 2.1% placebo.
Context: Jastreboff et al., NEJM 2023; NCT04881760. Primary endpoint week 24; week 48 was secondary. BMI >=30 or 27 to <30 with weight-related condition.
Limitations: Group estimates are not individual predictions. Excluded populations and longer-term maintenance or clinical outcomes are not established by this 48-week Phase 2 trial.
Route: SC · Once-weekly SC trial; absolute human SC bioavailability not reported in this efficacy publication.

Phase 2 weekly SC trial arms and titration
Six retatrutide randomized groups: 1 mg; 4 mg starting 2 mg; 4 mg starting 4 mg; 8 mg starting 2 mg; 8 mg starting 4 mg; and 12 mg starting 2 mg, plus placebo. Treatment continued 48 weeks.
Context: Jastreboff et al., NEJM 2023, NCT04881760. Study arms and initial doses from methods, not personal titration instructions.
Limitations: Research regimens are not recommended individualized protocols. This abstract does not specify each escalation step or validate a nontrial formulation.
Route: SC · Subcutaneous once weekly; oral, IM, split-weekly and combination routes/regimens were not evaluated.

Phase 2 safety and sensory adverse events
Gastrointestinal adverse events were most common, dose related and generally mild to moderate; lower initial dose partially mitigated them. Dose-related heart-rate increases peaked at 24 weeks and declined afterward. Supplemental Table S14 reports hyperesthesia-related events including sensitive skin, allodynia and skin-burning symptoms.
Context: Safety monitoring in 48-week Phase 2 obesity trial; supplementary adverse-event tabulation is source-distinct from trial abstract.
Limitations: Adverse-event reports show association, not a diagnosis or proof of causation for an individual. Rare and long-term risks remain incompletely characterized.

Phase 2a MASLD MRI-PDFF liver-fat endpoint
Relative reductions in liver fat at 24 weeks averaged 81.4% (8 mg) and 82.4% (12 mg); at 48 weeks, the 12 mg group averaged 86.0%.
Context: NCT04881760 MASLD substudy; 98 adults; MRI-based liver-fat endpoint rather than histologic fibrosis reversal.
Limitations: Small imaging substudy with reduced follow-up sample; cannot infer individual outcomes or fibrosis reversal, and liver-fat improvement does not isolate the glucagon-receptor mechanism.

Phase 2 evidence boundaries and research limitations
This trial does not establish long-term cardiovascular event reduction, safety of nontrial products, oral bioavailability, twice-weekly superiority, or benefit/safety of combining retatrutide with tirzepatide or other peptide therapies.
Context: 338-person Phase 2 placebo-controlled trial funded by Eli Lilly; no active semaglutide or tirzepatide comparator.

Context: Owner-published from the Jarvis queue onto Retatrutide. Evidence class U (Unresolved / evidence gap). This publication does not raise that class.
Limitations: Eli Lilly-funded Phase 3 trial; subgroup estimands differ from weight estimand; GI adverse events common; detailed full-text adverse event rates and PK not independently reviewed. Retatrutide phase 3 OSA findings — PMID 42904878
At week 80, placebo-adjusted AHI differences at 9 mg were -24.4 (TRIUMPH-1 OSA) and -14.4 (TRIUMPH-2 OSA) events/hour; at 12 mg, -21.9 and -15.1, respectively, under the hybrid estimand.
Context: Randomized placebo-controlled OSA subgroups: TRIUMPH-1 without diabetes n=243 and TRIUMPH-2 with diabetes n=448. The TRIUMPH-1 subgroup overlaps the earlier NEJM obesity paper and is not independent replication. Investigational once-weekly subcutaneous 4/9/12 mg arms studied over 80 weeks.
Limitations: Abstract-supported only. Full article, safety tables, attrition, escalation and statistical analysis plan not independently reconciled. Study doses are not recommendations; no approval or evidence-tier promotion implied.
Route: Subcutaneous in trial
